Case Coordinator

Assist consumers with identifying and articulating priority outcomes, personal goals and needs for services
and supports.  Develop and execute Implementation Plans/Individual Plans of Care.  Coordinate, monitor, and review consumer services.  Refer and link consumers to community resources as needed.  Maintain progress notes and treatment plans.  Advocate for consumer rights and benefits in courts settings when needed.  Establish a good rapport with consumers and family members.  Ensure that the delivery of services are implemented and documented correctly.  Contribute to the clients Individualized Educational Plan (IEP).


Residential Services
 
Supported Home Living – provided to individuals residing in their own residence or the residence of their natural or adoptive family; or to individuals receiving foster care services from the Texas Department of Protective and Regulatory Services.
 
Foster Companion Care Provider – is employed by the HCS provider and provides direct personal assistance with activities of daily living; assistance with housekeeping, meal planning and preparation; securing and providing ansportation; reinforcement of counseling and therapy activities; assistance with medications; facilitating inclusion in community activities, social interaction, participation in leisure activities and development of socially valued behaviors; and habilitation. These services are provided through supervision, assistance with grooming, bathing, toileting, tooth brushing, laundry, grocery shopping and general shopping.
 
Supervised Living – is provided in a residence in which the HCS program provider holds a property interest. Supervised Living is provided by residential assistance providers who provide services and supports as needed by individuals and who are present in the residence during normal sleeping hours.
 
Residential Support Services – this service allows the individual to reside in a group home setting receiving direct care services from trained 24-hour awake staff.

 

 

Specialized Services

Psychology – is provided by a licensed psychologist to individuals needing counseling or therapy or on a Behavior Therapy Program.

Occupational Therapy – therapy based on engagement in meaningful activities of daily life, especially to enable or encourage participation in such activities in spite of impairments or limitations in physical or mental functions.

Physical Therapy - the treatment or management of physical disability, malfunction, or pain by exercise, massage, hydrotherapy, etc., without the use of medicines, surgery, or radiation.

Speech Therapy - Treatment of speech defects and disorders, especially through use of exercises and audio-visual aids that develop new speech habits.

Social Work – provided by a licensed social worker to assistance with legal issues.


Health Services
Nursing provided by a LVN or RN to assist with monitor of the individual’s health status and consult with medical professionals regarding medication changes and help the individual understand their medical needs.

Audiology – provided when the doctor suspects that specialized hearing services or needed.

Dietary – provided by a dietician to assist with healthy meal planning or when the individual is on a special diet (diabetic diet, 1400 calorie diet etc..).

Dental – provided by a licensed dentist to assist with all dental needs. The waiver programs provides care costing up to $1000.00

Adaptive Aids – provided when the doctor and/or therapist believes that the individual requires a specific item to assistance with their adaptive living.


Other Services

Day Habilitation – assist individuals in acquiring, retaining, and/or improving self-help, socialization, and adaptive skills necessary to reside successfully in the community. Provide individuals with relevant activities that enhance self-esteem and maximize functional level. Reinforce skills or lessons taught in school, therapy, or other settings. Provide training and support activities which promote the individual’s integration and participation in the community. Provide assistance for individuals whom cannot manage their personal care needs during day habilitation activities. Provide transportation necessary for the individual’s participation in day habilitation activities.

Supported Employment - Supported employment is provided in an integrated provision of ongoing individualized support services needed to sustain paid work by the individual, including supervision and training. The individual receiving supported employment are compensated by the employer in accordance with the Fair Labor Standards Act. Supported employment is provided when these services are not available or funded through the state education agency or the state rehabilitation agency.

Minor Home Modifications – is provided when modification are needed in the home to benefit the individual or to improve overall living in the home for the individual. (bathroom rails, widening of doors etc.).

Respite – provided through the waiver program when the individual is receiving supported home living services to relieve the parents for planned or unplanned events.
